To add or make a change to your FMO affiliation, email the broker support team at hf-brokers@plusoscar.com with that request.

In an effort to maintain the integrity of the Health First Health Plans and AdventHealth Advantage Plans branding, prior written authorization from Health First is required for any and all logo use. The Broker, under no circumstances, will use Health Firsts' logo, branding, or trademark, whether for information or educational purposes, without collecting authorization first from Health First.
